In response to the last blog post we were emailed the following snapshot. In this unit over 20 different documents are used to make up the patient pack.
A well designed pathway document running from pre-assessment to discharge that included risk scores would make a massive improvement in this example.
An efficient enhanced recovery pathway which delivers high quality interventions every time will nearly always be supported by good documentation and paperwork.
How many different documents make up your enhanced recovery documentation?
Look out for instances of duplication, redundant sections, and poor flow of information.
